Chilled Semen Request Form

1. Please complete the form below to request a chilled semen collection. Only fully completed forms will be accepted.

2. We kindly request 24 hours notice of impending collection date. Please alert us via text message to 0437 722 441 that a new semen request has been submitted.

3. Semen will not be dispatched without prior payment. The cost of collection and freight is the responsibility of the mare owner or their agent.

4. Brent Eastwell & Associates Pty Ltd takes no responsibility for the semen once it has been placed in the hands of the couriers.

5. Please be cognisant of difficulties in arranging transport during or around weekends and public holidays. If unsure of your freight options, please contact us via text message to 0437 722 441 or email admin@beaequine.com.au. Preferred freight option can be requested in the Delivery Instructions field below, but availability cannot be guaranteed.

6. Privacy: Mare name, mare owner contact details, insemination centre details and pregnancy results will be shared with the stallion owner. No other personal information will be shared with any other party.
